Make a referral

Professional wanting to make a referral.

  1. An initial referral can be made through contacting the main switchboard who will gather the professional's details and refer them on to the Clinical Team

  2. The Clinical team will discuss with the professional the needs of the individual for a referral.

  3. The professional will be advised of  the fees of all the programmes

  4. A referral form is available for professionals for all programmes and this can be emailed or posted out to the professional 

  5. Professional will be advised that individuals can make a self-referral also
